Whether you've already requested your medical records or are still looking for a new healthcare provider, you are warmly invited to stay with us. You're more than welcome to continue receiving care at this same convenient location under Dr. Lee's expert care.
You do not need to take any special action to remain with our practice. Whenever you need an appointment or medical care for any reason, simply call our office as you normally would, and we'll take care of the rest. The only thing we ask is the short acknowledgment form below.
To make this transition as seamless as possible, our manager and Medical Assistant, Ms. Jae Yom, will remain on site as part of the team to support Dr. Lee and assist you with the same warm, familiar service you have always known.
Complete this short acknowledgment form confirming that Dr. Lee will continue your child's care and that our current office policies apply — that's it, no other paperwork needed.
